Our Mission
The Illinois Continuity of Care Association exists to enhance the concept of continuity of care as both as essential part of the health care continuum and as integral to total patient care. We advocate for legislative and community-related health concerns and issues by identifying and communicating them to the members, collaborating with allied organizations, and supporting standards of professional competence which identify, structure, and promote a multi-disciplinary approach to continuity of care.

Educate
IL-COC serves as an information resource for consumers and care providers. Our resources include informational updates and a searchable directory of members statewide who provide continuity of care services. Our members represent over 50 senior service provider categories. These include Community Resources, Elder Law Attorneys, Move Management Companies, Senior Real Estate Specialists, Geriatric Care Managers, Home Health Agencies, Non-Medical Home Health, Hospice Experts, and all levels of Senior Housing: Independent, Assisted, Memory Care, and Skilled Nursing Facilities.

Advocate
Both independently, and in collaboration with allied organizations, IL-COC advocates at legislative & community levels throughout the state on behalf of providers and care recipients. We identify healthcare issues and initiatives. We communicate these to members via this website, regular email alerts, our monthly chapter meetings, after-hours events, and ongoing educational seminars.

Care
We work to ensure that older Illinois adults and individuals with disabilities who have ongoing health-related needs receive high-quality care and support after exiting the healthcare system. Key efforts include supporting standards of professional competence to promote a coordinated multi-disciplinary approach to continuity of care.
